BFM B-Cell

Regimen

Prephase

Cyclophosphamide 200mg/m2 IVI 1 hr days 1-5
Prednisolone 60mg/m2 PO days 1-5

Block A

Ifosfamide 800mg/m2 IVI 1 hr days 1-5
Vincristine 1.4mg/m2 IVB day 1
Methotrexate 1gm/m2 iv infusion over 24 hours day 1
Folinic acid rescue must start exactly 18 hours upon completion of methotrexate. Folinic acid (Leucovorin) 25mg/m2 oral 6 hourly x 8 doses, then  15 mg/m2 6 hourly x 8 doses. 
Intrathecal methotrexate 12.5mg and Ara C 40mg  day 1,5
VM26 100mg/m2 iv infusion over 1 hour day 4,5
Ara C 150mg/m2 iv infusion over 1 hour bd day 4,5
Dexamethasone 10mg/m2 PO days 1-5

Block B

Cyclophosphamide 200 mg/m2 iv infusion over 1 hour  days 1-5
Vincristine 1.4mg/m2 IVB day 1
Methotrexate 1gm/m2 iv infusion over 24 hours day 1

Cycles alternate between Block A and Block B q 21-28 days x 6 cycles
