Regimen

 Fludarabine  30 mg/m2 intravenous (i.v.) infusion for 30 min on days 1 to 5
 Ara-C 2 g/m2 i.v. infusion (started 4 h after the start of the Fludarabine) for 2 h on days 1 to 5 
 G-CSF (filgrastim) 300 μg s.c. from day 0 until neutrophil recovery
 Idarubicin 8 mg/m2 i.v. infusion for 30 min on days 1 to 3
 *Idarubicin is omitted in some cases, by physician's discretion
 Bacterial, fungal, PCP and viral prophylaxis
