M-BACOD

Regimen

Methotrexate 200 mg/m2 IV on days 8 and 15
Leucoverin 10 mg/m2 PO every 6 hours for 8 doses beginning 24 hours after methotrexate
Bleomycin 4 u/m2 IV on day 1
Doxorubicin 45 mg/m2 IV on day 1
Cyclophosphamide 600 mg/m2 IV on day 1
Vincristine 1 mg/m2 (max 2 mg) IV on day 1
Dexamethasone 6 mg/m2 PO on days 1-5

Repeat cycle every 21 days
