Stanford V

Regimen

 Vinblastine IV 6 mg/m2 on week 1, 3, 5, 7, 9, 11
 Doxorubicin IV 25 mg/m2 on week 1, 3, 5, 7, 9, 11
 Vincristine IV 1.4 mg/m2 (max 2mg) on week 2, 4, 6, 8, 10, 12
 Bleomycin IV 5 U/m2 on week 2, 4, 6, 8, 10, 12
 Mustard IV 6 mg/m2 on week 1, 5, 9
 Etoposide IV 60 mg/m2 daily for 2 days on week 3, 7, 11
 Prednisolone Oral 40 mg/m2 EOD from week 1 to week 10, taper over last 2 weeks
 G-CSF for 5 days on odd weeks

 Prophylaxis with Co-trimoxazole, Acyclovir and Ranitidine

 36Gy RT to initial disease > 5cm and macroscopic splenic disease
 Bulky mediastinum - modified mantle field RT, incudes mediastinal, bilateral hilar and bilateral low cervical LN
