Regimen

MINE

Ifosfamide 1333 mg/m2 IV (1 hr) days 1-3
with Mesna
Mitoxantrone 8 mg/m2 IV (15 min) day 1
Etoposide 65 mg/m2 IV (1 hr) days 1-3

To be repeated every 3 weeks. For a maximum of 6 courses followed by

ESHAP

Etoposide 60 mg/m2 IV (1 hr) day 1-4
Methylprednisolone 500 mg/m2 IV (15 min) days 1-4
Cytarabine 2000 mg/m2 IV (2 hr) day 5
Cisplatin 25 mg/m2 IV (CI) days 1-4

To be repeated every 3 weeks.
For 3 courses to consolidate a complete response or for a maximum of 6 courses after a partial response or no response to MINE
